JACKSONVILLE, Fla. (AP) – The Florida Department of Children and Families has filed to revoke the license of a Jacksonville child care facility after a 6-year-old left the building without the knowledge of anyone inside.

The Florida Times-Union reports the agency’s investigation into the January incident found two other violations at Kidz n Kidz Academy.

A report indicates the facility had insufficient supervision and a licensing counselor found five children inside a closet during a head count of children. The report says the facility’s director didn’t deny kids were put in the closet to keep them from being counted by DCF.

Agency spokesman John Harrell told the newspaper that any facility with three violations in two years is subject to having its license revoked.

The agency has until April 12 to respond.
